Do I qualify for Value Health insurance?
The Life Insurance Company of the City of New York issues "Value Health" or "value Health USA" individual insurance through "Value Benefits Association" to individual applicants who meet the residency and medical eligibility requirements. Before pricing coverage or submitting an application, please review the eligibility requirements listed below.
Applicants must be a resident of any state EXCEPT Connecticut, Kentucky, Louisiana, Maine, New Jersey, New York, Oregon, Utah or Washington. U.S. citizenship is not required.
Medical Eligibility Requirements
Medical underwriting criteria is based on medical conditions and treatment within the past 24 months. Medical conditions or treatment obtained more than 24 months ago do not affect eligibility.
Applicants who can answer "NO" to the following two medical questions are automatically accepted:
1. In the past 24 months have you or your dependents, if applying for
insurance, had chest pains, disease or disorder of the heart, liver, kidneys or
lungs, high blood pressure, albumin, or sugar in the urine, diabetes, cancer,
tumors or ulcers?
2. Have you or your dependents, if applying for insurance, consulted any physician or practitioner for any reason other than a routine physical exam with normal results, or been confined or treated in a hospital or similar institution during the past 24 months?
Those who answer "YES" should continue below to determine their eligibility:
Applicant's with the following ongoing or uncontrolled medical conditions or treatment within the past 24 months are not eligible for coverage: Addison's disease, alcoholism, alzheimer's disease, amputation due to disease, amyotrophic lateral sclerosis, anemia, arteriosclerosis obliterans, severe or crippling arthritis, hospitalization for blood pressure, chronic bran syndrome. brain tumor, Bright's disease, chronic nephritis, cardio-renal disease, cerebral palsy, cerebro-vascular disease or disorder, chronic obstructive lung disease, club foot, colostomy, convulsions, coronary artery disease, Crohn's disease, Cushings disease or syndrome, cystic fibrosis, diabetic neuropathy, Down's disease, drug abuse, grand mal epilepsy, heart pacemaker, hemophelia, hepatitis, Huntington's Chorea, hydrocephalus, hydronephrosis, kidney failure, kidney removal, leukemia, liver disease, lung disease, lupus, mental retardation, multiple sclerosis, muscular dystrophy, myasthenia gravis, hospitalized or institutionalized or disabled nervous mental disorders, organ transplant (other than cornea), Paget's disease, pancreatis, paraplegia, Parkinson's disease, pregnancy, quadriplegia, Raynaud's disease/phenomena, spina bifida, degenerative spine disease, ulcerative colitis, heart valve replacement, wheelchair or walker required for movement.
Applicants with other medical conditions or treatments within the past 24 months are eligible for coverage if the medical condition is controlled or resolved.
